Born, Loved, and Never Forgotten:
A State-By-State Guide
A Certificate of Birth Resulting in Stillbirth - sometimes called a Certificate of Life Resulting in Stillbirth or a similar name depending on the state- is an official document that recognizes a baby who was stillborn. While it is not a legal birth certificate, it provides acknowledgement that a baby was born and that their life mattered.
Sadly, many bereaved parents never learn this certificate exists. In many cases, hospitals and even health care providers are unaware that their state offers one, leaving families to discover it months or even years after their loss. We hope this resource helps parents find out whether their state provides this meaningful recognition and how to apply for it.
Every baby deserves to be remembered, and every family deserves to know the resources available to honor their child.
Here, you’ll find state-by-state information to help you understand what certificates may be available, who is eligible to receive them, and how to request them.
Find your state below to find the most up-to-date information, including application details, helpful links, and any special requirements. Our hope is that having these resources gathered in one place allows you to spend less time searching for answers and more time honoring the memory of your precious baby.
